Information on initial examinations
If you are a new patient and want to book an initial examination, you must register at the office at least a month in advance!
Before the initial medical examination:
1. Book an appointment at the clinic for a specific date and time using the form "Objednat se".
2. Please print out the questionnaire for your initial examination, fill it out, and bring it with you to the office for your blood draw; alternatively, you may send it via email in advance. You can find the questionnaire in the downloadable materials. >>ZDE<<
3.In the week leading up to your initial examination, please come to the office for blood draws and bring a urine sample. Blood draws take place daily—except for Thursdays—between 7:00 and 8:00 AM. You must arrive on an empty stomach; you may drink only plain water.
